This  series of online courses gives Case Managers the knowledge and skills needed to assess needs of older adults, monitor multiple chronic conditions, and educate and empower patients to improve health and quality of life.  The goal is to transform chronic care for high-risk adults by enabling health care teams to provide proactive, coordinated, comprehensive care for such patients.

(Courses in this series were developed primarily for nurses practicing the Guided Care model on older adults.  They may mention Guided Care, however, the principles are applicable to J-CHiP, specifically the Medicare population.)
