Transforming Chronic Care for Older Adults: Series for J-CHiP Health Behavior Specialists
This series of online courses is designed to provide knowledge about specific conditions (diabetes, delirium, dementia, hypertension) and elder abuse to help Health Behavior Specialists understand their effects on older adults in the Johns Hopkins Community Health Partnership (J-CHiP). These courses should be viewed by professionals in a healthcare setting where these issues are prevalent. The goal is to transform chronic care for high-risk adults by enabling health care teams to provide proactive, coordinated, comprehensive care for such patients.
(Courses in this series were developed primarily for nurses practicing the Guided Care model on older adults. They may mention Guided Care, however, the principles are applicable to J-CHiP, specifically the Medicare population.)
