After completing the activities in Unit III, the learner will be able to: identify the processes that constitute ongoing Guided Care; discuss strategies and resources for supporting patients and caregivers in the community; conduct a Monitoring and Coaching call with a Guided Care patient; and discuss strategies for effective management of transitional care.
