Registered nurses (RN) and healthcare providers focusing on best practices in chronic disease management, case management, caregiver education and support, transitional care, and geriatric evaluation and management.

Learning Objectives

  • Describe the process by which nurses complete the components of the "Health History."
  • List three examples of possible entries in the "Nature" and "Reason" columns in the "Chronic Medications" section of the "Health History."
  • List the findings in the patient's general status assessment that would trigger more detailed assessment of specific conditions.

Michele D. Phillips, BSN, RN, CCM
Michele Phillips is a pioneer in Guided Care Nursing.  She joined Johns Hopkins Healthcare in 2006 as a Guided Care Nurse, one of the first nurses to retain this position.  A participant in the Johns Hopkins School of Public Health study on Guided Care Nursing, Ms. Phillips works closely with a geriatric population of over 50 patients providing coaching and motivational interviewing, transitional care, health assessments, and collaboration with caregivers and other healthcare providers.  She received a special gerontology award for her knowledge and exceptional sensitivity and caring for the older adult population. 
Her extensive clinical nursing background includes experience in operating room, progressive and intensive care and travel nursing.  Ms. Phillips is certified in case management and is an active member of the Case Management Society of America and the National Gerontology Nursing Association.
